By using this website or submitting an appointment request, you agree to these terms. If you do not agree, do not use the website. These terms do not create a practitioner-patient relationship.
Website Information
Website content is provided for general educational and administrative purposes. It is not a diagnosis, individualized medical advice, emergency guidance, or a promise of results. Acupuncture and wellness care are individualized, and outcomes vary.
Appointments and Communications
An online form submission is a request, not a confirmed appointment. An appointment is confirmed only after direct communication from Jacob AcuBalance. Please use 911 or appropriate emergency services for emergencies.
SMS Messaging Terms
Program name: Jacob AcuBalance Appointment Communications.
Program description: When you separately check the optional, unchecked SMS consent box on our appointment request form, you agree to receive informational text messages from Jacob AcuBalance at the mobile number you provide. Messages may include responses to appointment requests, appointment confirmations, reminders, rescheduling notices, and clinic-related scheduling updates. Promotional or marketing messages are not included in this consent.
Message frequency: Message frequency varies based on appointment activity and requests and is typically no more than four messages per appointment request or scheduled visit.
Charges: Message and data rates may apply.
Opt out: Reply STOP to unsubscribe at any time. After opting out, you may receive one final confirmation message. To resume messages later, you must provide a new affirmative opt-in.

Consent: Consent is voluntary and is not a condition of treatment, requesting an appointment, or purchasing any service. The SMS checkbox is optional and is not selected by default.

Privacy: No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text-messaging originator opt-in data and consent will not be shared, sold, rented, transferred, or otherwise provided to any third parties, affiliates, lead generators, or other organizations for marketing or promotional purposes. Acceptable Use
You may not misuse the website, attempt unauthorized access, introduce malicious code, scrape personal information, interfere with website operation, or submit false or unlawful requests.
Third-Party Services
The website may connect to services such as Formspree, Google Analytics, Google Maps, email, phone, and messaging providers. Their services are governed by their own terms and policies.
Intellectual Property
Website design, text, logos, and original content are owned by Jacob AcuBalance or used with permission. They may not be copied or republished without authorization, except for personal, noncommercial use.
Limitation of Liability
To the extent permitted by law, Jacob AcuBalance is not responsible for damages arising from reliance on general website information, website interruption, third-party services, or unauthorized access beyond our reasonable control.
